When is Allergy Testing Appropriate?

Testing done by a doctor is generally safe and effective for adults of all ages. The allergen extracts or vaccines used in allergy tests performed by us meet U.S. Food and Drug Administration (FDA) requirements.

Symptoms which usually prompt an allergist to perform testing include:

Respiratory: itchy eyes, nose or throat; nasal congestion, runny nose, watery eyes, chest congestion, cough or wheezing
Skin: itchiness or eczema
Abdominal: vomiting or cramping and diarrhea consistently after eating certain foods
Severe reactions to stinging insect stings (other than swelling at the site of the sting)
Anaphylaxis (pronounced an-a-fi-LAK-sis): a serious allergic reaction that affects many parts of the body at the same time.

It is important that allergy testing is directed by a healthcare professional with sufficient allergy/immunology training and prompted by your medical history.

Types of Allergy Tests

Different allergens bother different people, so your allergist will determine which test is the best for you. Regardless of the type of test, an allergist will first perform a physical examination and ask questions about your symptoms to determine if allergy testing is warranted.

IgE Skin Tests

This type of testing is the most common and is relatively painless. A very small amount of certain allergens is put into your skin by making a small indentation or “prick” on the surface of your skin.

If you have allergies, just a little swelling that looks and feels like a mosquito bite will occur where the allergen(s) to which you are allergic was introduced. If you are allergic to ragweed pollen but not to cats, only the ragweed allergen will cause a little swelling or itching. The spot where the cat allergen was applied will remain normal.

You don’t have to wait long to find out what is triggering your allergies. Reactions occur within about 20 minutes. And you generally won’t have any other symptoms besides the small hives where the tests were done, which go away within 30 minutes. If your prick skin tests are negative but your physician still suspects you might have allergies, more sensitive “intradermal” tests may be used in which a small amount of allergen is injected within Skin tests are best performed in an allergist’s office to assure the test results are read properly and to minimize the risk of rare side effects.

Allergy skin testing is an in-office procedure used to identify specific allergens that may be causing your symptoms. A small amount of common North East Texas allergens is placed on the skin using a gentle prick method. If you’re sensitive to any of these allergens, a small raised bump may appear at the test site. The results help your provider determine which environmental triggers are contributing to your reactions and guide your treatment plan.

Immunotherapy (Allergy Shots)

Allergen immunotherapy is a long-term treatment designed to help reduce your sensitivity to the allergens identified during testing. Based on your skin-test results, a customized blend of allergens is prepared for you. Small, gradually increasing doses are administered in the clinic on a regular schedule. Over time, immunotherapy can help lessen the frequency and severity of allergic symptoms by training your immune system to respond less strongly to these triggers.

Immunotherapy

Anyone who has lived in the North Texas area has experienced allergies at some point in their lives. Our area has amongst the highest pollen counts in the nation. Patients with allergies constantly suffer from sneezing, runny nose, watery eyes and have multiple ear, sinus and bronchial infections.

Treatment for seasonal and perennial allergies consist of continuous use of several antihistamines, steroid nasal sprays and inhalers. However these medications give limited relief for the most part, and there is also significant fatigue having to take these medications continuously.

Allergen immunotherapy , also called allergy shots, comes to the rescue in this situation. Immunotherapy is a preventive treatment for allergies from environmental agents such as pollen, grasses, trees, mold and dust mites. Allergy shots make the immune system to be less sensitive to these environmental agents, and they reduce inflammation in the respiratory tract that causes allergy symptoms.

Tapia Internal Medicine Clinic is now offering a full scale of Allergy skin testing and Allergen Immunotherapy to our area where these options are limited but very needed.

The Allergen Immunotherapy program consists of the following steps.

Immunotherapy can be given in
2 ways.

Subcutaneous Immunotherapy: In subcutaneous immunotherapy the allergen is administered in the form of allergy shots. Allergy shots are given with increasing amounts of allergens once a week to help your body become immune to the allergen. After about 3-6 months of therapy you will notice a significant decrease in your symptoms of allergies.Allergy shots are given in the doctor’s office so that we can monitor you for any adverse reactions.

Sublingual Immunotherapy: In sublingual immunotherapy the allergen is administered in the form of drops which are taken under the tongue. These are very well tolerated and offer the convenience of using them at home and not having to come to the office once a week. These are not covered by insurance, however are affordable pricing of just $20 a week makes it possible for patients to utilize this therapy.

Type 2 diabetes

Is there a test for diabetic retinopathy?

Yes, here at Tapia Internal Medicine Clinic, PLLC the physicians perform a diabetic eye exam. This includes the physician giving you eye drops to make your pupils open up. (The drops make it easier for the doctor to see the different parts of the inside of your eye.) After the drops have done their job, the doctor will take pictures of the eye with a special camera. If the pictures look normal then they will recheck the eyes in a year. If the pictures look abnormal, the patient is notified and a referral to an eye doctor will be created for further examination.

How is diabetic retinopathy treated?

When mild, diabetic retinopathy is not always treated. But people with the condition do need to keep their blood sugar and blood pressure levels as close to normal as possible. This will help the condition from becoming worse.

Treatments for diabetic retinopathy can include:

Photocoagulation- This is laser surgery to seal or destroy leaking or growing blood vessels in the retina.
Vitrectomy- This is surgery to remove blood from the part of the eye called the “vitreous humor”. Doctors do this in surgery if the blood vessels in the retina leak into the vitreous humor.
Medicines- Medicines that are injected into the vitreous humor are sometimes used along with photocoagulation or vitrectomy. Your eye doctor will let you know if this type of treatment will help you.

Can diabetic retinopathy be prevented?

Yes. If you have diabetes, you can reduce your chances of getting diabetic retinopathy by keeping your blood sugar and blood pressure level as close to normal as possible. It might also be important to keep cholesterol levels in the “normal” range.

Why is having a dilated eye exam important for someone with diabetes?

A dilated eye exam allows the physician to examine the back of your eye in detail, helping detect early signs of diabetes-related eye changes (like retinopathy or macular edema) before you notice vision problems. Routine detection means your care plan can be adjusted sooner to help preserve your sight.

How is the evaluation conducted for older adults at your clinic?

What is high cholesterol?

High cholesterol is determined by your total cholesterol number from your blood test.

Desirable: Less than 200 mg/dl
Borderline high: 200 mg/dl to 239 mg/dl
High: Greater than 240 mg/dl

Total cholesterol count includes two types of cholesterol: low-density lipoproteins (LDL), referred to as bad cholesterol, and high-density lipoproteins (HDL), referred to as good cholesterol. In addition to your total cholesterol, your physician wants you to pay attention to your LDL and HDL numbers.

For better heart health:

LDL less than 100 mg/dl
HDL greater than 40 mg/dl

What is cholesterol?

Cholesterol is a substance that is found in the blood. Everyone has some. It is needed for good health. The problem is, people sometimes have too much cholesterol. Compared with people with normal cholesterol, people with high cholesterol have a higher risk of heart attacks, strokes and other health problems. The higher your cholesterol, the higher your risk of these problems.

Should I take medicine to lower my cholesterol?

HYPERTENSION

Hypertension, also known as high blood pressure, is a condition that puts you at risk for heart attack, stroke and kidney disease. It does not usually cause symptoms. But it can be serious. The physicians here at Tapia Internal Medicine Clinic, PLLC want to educate you on what hypertension is as well as ways to treat and prevent it.

When your physician or nurse tells you your blood pressure, he or she will say 2 numbers. For instance, “140 over 90”. The top number is the pressure inside your arteries when your heart is contracting. The bottom number is the pressure inside your arteries when your heart is relaxed. When left untreated, hypertension increases your heart’s workload and damages the delicate tissue of your blood vessels, increasing your risk of heart attack, kidney failure and stroke. “Elevated blood pressure” is a term doctors or nurses use as a warning. People with elevated blood pressure do not yet have high blood pressure. But their blood pressure is not as low as it should be for good health.

How do I know if I have hypertension?

Hypertension is often referred to as the silent killer because there are no symptoms, which is why the physicians at Tapia Internal Medicine Clinic, PLLC check your blood pressure every time you come to see them.

Normal blood pressure: Less than 120 mm Hg/80 mm Hg
Elevated blood pressure: 120-129 mm Hg/80 mm Hg or less
High blood pressure: 140 mm Hg or higher/90 mm Hg or higher

Keep in mind: A single episode of high blood pressure doesn’t mean you have hypertension. If you have an elevated reading, the doctor monitors your numbers at your regular visits and may have you check at home using a home testing machine or at a blood pressure testing machine at your local pharmacy.

If your blood pressure is consistently high, the physician may diagnose you with hypertension.

How can I lower my blood pressure?

If your doctor or nurse has prescribed blood pressure medicine, the most important thing you can do is to take it as prescribed. If it causes side effects, do not just stop taking it. Instead, talk to your doctor about the problems it causes. He or she might be able to lower your dose or switch you to another medicine. If cost is a problem, mention that too. Your doctor might be able to put you on a less expensive medicine. Taking your blood pressure medicine can keep you from having a heart attack or stroke, and it can save your life.

Can I do anything on my own?

You have a lot of control over your blood pressure. To lower it:

Lose weight (if you are overweight)
Choose a diet low in fat and rich in fruits, vegetables and low-fat dairy products
Reduce the amount of salt you eat
Do something active for at least 30 minutes a day on most days of the week
Cut down on alcohol (if you drink more than 2 alcoholic drinks per day)

It’s also a good idea to get a home blood pressure meter. People who check their own blood pressure at home do better at keeping it low and can sometimes even reduce the amount of medicine they take.

What does “normal” vs “high” blood pressure mean?

Normal blood pressure is under 120/80 mm Hg, while a reading of 140/90 mm Hg or higher may indicate hypertension. A single high reading doesn’t always mean hypertension—it’s the persistent pattern that matters.

Can I monitor my blood pressure at home, and is that useful?

Yes. Having a home blood pressure monitor can help track your numbers between clinic visits, identify trends, and give us more data to guide treatment. It also helps in detecting “white coat” effects (higher readings at the clinic).

If I’m on blood-pressure medication, do I still need lifestyle changes?

Who performs the evaluation at Tapia Clinic?

At Tapia Clinic, your evaluation is carried out by one of our internal-medicine physicians. They review your health history, perform required testing, and collaborate with your surgeon if needed.

What kinds of tests might be required as part of the evaluation?

Depending on your health and the planned surgery, we may request an electrocardiogram (EKG), lab work (blood tests), imaging (X-ray, CT or MRI), screening questionnaires, and review of conditions like diabetes, hypertension, cholesterol and allergies.

How far in advance of my surgery should I schedule the evaluation?

It’s best to schedule the evaluation as soon as your surgeon requests it or when the surgery is confirmed. The earlier we do it, the more time we have to identify issues and coordinate care without delaying your surgery.

What happens if the evaluation finds a medical issue?

If we identify a condition that needs attention (for example uncontrolled blood pressure or new heart rhythm findings), we will work to stabilize or manage it before surgery and communicate recommendations to your surgeon. Our goal is to improve safety and reduce surgical risk.

Will this evaluation replace the visit to my surgeon or other specialists?

No. The evaluation complements your surgical care but does not replace the surgeon’s assessment or any specialty consultations you may need. We focus on your overall health and readiness for surgery, while your surgeon focuses on the procedure.

Do I have to fast or stop medications before the evaluation?

Not always. For routine evaluation visits we usually do standard lab tests and an EKG, which may require overnight fasting. Your provider will give you specific instructions. Also, please bring a list of all current medications so we can review them in the context of your upcoming surgery.

A physical examination is a routine test your primary care provider performs to check your overall health. There are different tests that can be performed during your physical examination. Depending on your age or medical or family history, your PCP may recommend additional testing.

What is preventive care?

Preventive care is the care you receive to prevent illness and/or disease. It also includes counseling to prevent health problems.

What is an Annual Wellness Visit (AWV)?

The annual wellness visit is a yearly appointment with your primary care provider to create or update a personalized prevention plan. This plan may help prevent illness based on your current health and risk factors. Keep in mind that the AWV is not a head-to-toe physical.

How often should I have a preventive-care visit if I’m healthy?

For generally healthy adults without major risk factors, an annual preventive visit is typically recommended to maintain wellness, update screenings, and catch any changes early.

Why do I need Vaccinations?

What is a well-woman exam?

A well-woman exam is a complete physical that includes an examination of your reproductive organs. During the exam the physicians at Tapia Internal Medicine Clinic PLLC will obtain your vitals, including heart rate, blood pressure, height and weight. The provider will then examine your body from head to toe, looking into your eyes and ears, feeling your organs and checking your reflexes. This will also include a pelvic exam.

Why would I need a Pap smear?

The Pap smear checks for abnormalities in the cells of your cervix. Abnormal cells may be a sign of cervical cancer. Detecting changes early may improve the success of treatment.

What is a breast exam?

Clinical breast examinations can detect a lump in your breast and other changes that might require more testing. Breast exams are one of the most important early breast cancer screenings. If more testing is required your physician may order a mammogram. A mammogram is an x-ray picture of the breast.

Do you help with contraception counselling and hormone replacement therapy (HRT)?

Why do I need a bone density scan?

Osteoporosis is a common disease that is characterized by low bone mass, microarchitectural disruption, and skeletal fragility, resulting in an increased risk of fracture. The goal of bone density screening is to identify persons at increased risk of sustaining a low-trauma fracture who would benefit from intervention to minimize that risk.

What are acute gynecological medical concerns?
